Do I need malaria tablets?

Malaria is a serious disease and can be fatal so if you are travelling to a location where it is present, you should get a course of malaria tablets as soon as possible before travelling. Malaria can be more severe for pregnant women, old people and children.

How do you get malaria?

Malaria is contracted through a mosquito bite which carries a parasite called plasmodium. If the mosquito is infected, it injects this parasite into your blood. These mosquitoes mostly bite at night between bedtimes until dawn.

What are the different types of malaria?

There are 3 main kinds of malaria are categorised by their development cycle and the frequency of their symptoms.

  • Tertian malaria leads to a fever which returns every three days hence the name “tertian”. This category covers one of the most severe types of malaria called Plasmodium falciparum, although plasmodium falciparum does not abide by the three day rule, it can occur randomly or sometimes show no symptoms for a long period.
  • Quartan malaria leads to a fever which occurs every fourth day hence the name “quartan”.
  • Malaria tropica is the third kind of malaria which is often confused with a cold or flu due to random and less indicative symptoms.

Where is malaria usually found?

Malaria is present and recorded in over a 100 different countries, majority of the time in tropical regions. Some of these areas include major parts of Africa, Asia, and Central America, some parts of the Middle East and a few pacific islands like Papua New Guinea. Malaria is not an issue found in the UK.

What to do if you are bitten?

Malaria is curable using anti-malaria tablets but treatment has to start imminently. Therefore it is wiser to actually prevent getting malaria in the first place. There are currently three types of malaria tablets licensed in the UK. The most popular and most expensive is Malarone which contains the active ingredients Atovaquone and Proganil. Malarone is available as a generic at a slightly lower cost, both Malarone and generic malarone are clinically identical and equally as effective. The other popular antimalarials are Lariam which contains Mefloquine and finally the most sold malaria tablets from DrFelix is Doxycycline. Doxycycline is the cheapest out of the three and highly effective, but before you order doxycycline online you must check the Fit for travel website and make sure you are not allergic to any of the tablets.

How does the malaria treatment work?

Anti-malaria treatment works by elimination the parasites in your blood stream which is injected through a mosquito bite. You must follow the dosage instructions for malaria tablets to remain protected.

Is malaria contagious?

Malaria is not contagious between humans; it is mostly spread through mosquito bites. There is a small possibility it can be transferred between humans through blood transfusion if one of them is infected since the parasite is carried in the red blood cells. Malaria can also be transmitted to new born babies if the mother is infected during pregnancy.

Is there a vaccine for malaria?

What are the symptoms of malaria?

Malaria symptoms can include a headache, fever, being sick, similar symptoms to a flu or a cold, sweats and chills, sore muscles and diarrhoea. Some people will see symptoms appear within a day whilst others may take a week to a month, in some rare cases even up to a year.

The initial symptoms of malaria can be mild so it is difficult for GPs to diagnose malaria as the patterns for different malaria types can vary.

What is the incubation period for malaria?

Due to the variation in development of the parasites, the incubation period is not the same. Some parasites can even develop within 24hours whilst other can take a week or between 10days to a month. The development time period of the parasite correlates with the incubation periods.

Doxycycline 100mg

For the prevention of malaria, Take one capsules of Doxycycline 100mg daily. Start the capsules 2 days before the trip and take one capsule everyday while you are in a malaria risk area, followed by continued treatment for four weeks after you have left the risk area. Doxycycline is highly effective when taken correctly and is the cheapest malaria tablets from the rest.

Lariam or Mefloquine

For the prevention of malaria, the adult dosage is to take one Lariam or Mefloquine tablet weekly. Lariam is started 3 weeks before the trip, continued for the duration of the trip and then finished after a four week course after you have left the risk area.

Malarone / Generic Malarone (Atovaquone plus proguanil)

For the prevention of malaria, Malarone for adults is one tablet daily and varies for children. DrFelix does not supply malarone tablets for children. Malarone should be started 2 days before your trip, and then continued daily followed by a full week once you leave the malaria risk area. Malarone is the most expensive out of malaria tablets so you may not consider it for longer trips.

Doxycycline

Doxycycline is not recommended for children under 12 and pregnant or breastfeeding women. If you have liver problems or are sensitive to the tetracycline antibiotic then you should also avoid doxycycline. It is also known to reduce the effectiveness of the contraceptive pill or patches.

Doxycycline side effects can include an upset stomach, thrush, heartburn and possible sunburn due to sensitivity to sun.

Lariam (Mefloquine)

Lariam / Mefloquine are not recommended for people with heart or liver problems. It is also not recommended for people with epilepsy, seizures or any mental health problems like depression. Please advise your doctor during the consultation if you suffer from any of these conditions before taking Lariam.

Lariam side effects can include a headache, dizziness, insomnia, panic attacks or hallucinations and anxiety or depression.

Malarone / Generic Malrone (Atovaquone plus proguanil)

Malarone is not recommended for pregnant women due to its unknown side effects. Anyone with kidney problem should also avoid atovaquone plus proguanil.

Malarone side effects can include a skin rash, headache, some mouth ulcers and an upset stomach.

Warnings!

If you experience any flu symptoms after your trip, seek medical help immediately. This is due to a type of malaria parasite which can show similar symptoms to a flu or a cold and can be serious.
If you are doing trekking, especially in remote areas with no access to medical assistance, keep your course of malaria tablets with you. If you get bitten the treatment has to be started immediately. You will be advised more about this when you consult a travel clinic before you travel abroad.

Common use

Doxycyline is a synthetic broad-spectrum antibiotic derived from tetracycline and used to treat many different bacterial infections, such as infections of urinary tract, acne, gonorrhea, chlamydiosis, periodontitis (gum disease), blemishes, bumps, and acne-like lesions caused by rosacea.

Dosage and directions

This medication should be taken after a meal with a big glass of water to decrease irritation of esophagus. Daily dose should be taken as a single dose or divided in two (12 hours between each one). Do not take it in larger amounts or continue to take it after the administered period. Certain brands of this medication cannot be used with dairy products. Do not crush, break, or open a delayed-release capsule, eat the pill whole. If it is an oral suspension shake it before use and measure with special measuring spoon. Never take this drug if it is expired as it may damage your kidney.

Precautions

Usage of Doxycycline during growth of teeth may result in their permanent color change. Avoid exposure of your body to prolonged UV radiation during treatment and 4-5 days after it. If long treatment with this medication is prescribed a regular monitoring of peripheral blood cell content, liver function tests and determination of nitrogen and urea in serum are required.

Contraindications

Hypersensitivity to Doxycycline and tetracyclines, pregnancy (second half of the term), breastfeeding, infancy (for children younger than 9 y.o.), leucopenia, porphyria, severe liver failure.

Possible side effect

Side effects caused by Doxycycline may appear as headache, dizziness, blurred vision, fever, chills, body aches, red skin rash, pale or yellowed skin, dark colored urine, severe pain in upper stomach, loss of appetite, jaundice, easy bruising or bleeding. Contact your doctor if you experience severe effects listed above.

Drug interaction

Co-administration with aluminum, magnesium, or calcium based antacids is not recommended as Doxycyline absorption maybe slowered. Minerals (such as calcium or iron) or with bismuth subsalicylate have the same effect. Doxycycline is reported to enhance the activity of warfarin. Phenytoin (Dilantin), carbamazepine (Tegretol), and barbiturates (such as phenobarbital) may enhance the metabolism of Doxycycline.

Missed dose

Never take a double dose of this medication. If it is almost time of the next dose just skip the missed portion and continue to take the medicine according to the schedule.

Overdose

In case of serious and persistent nausea, vomiting, and diarrhea consult your doctor.

Storage

Store at room temperature between 59-77 degrees F (15-25 degrees C) away from light and moisture, kids and pets. Do not use after expiration term.
